Community Integration- We provide our clients with support based on their individual needs. Community Integration may include one or all of the following: brief community integration, case-management, advocacy, life planning, linking to services, skills teaching, consultation and problem solving, and development of Individual Support Plans.

Community Rehabilitation Services (CRS) Program - The goal of the service is to assist individuals in being able to remain a productive member of their community as well as to maintain as much independence in their daily lives as possible. CRS can be utilized as a flexible and comprehensive support for people as they leave a hospital or other higher levels of care. CRS blends the components of Community Integration, Daily Living Skills Support and Skill Development into a new, distinct service. Length of service will be determined by individual clinical need.
